Patient Info

To Our Patients:

We appreciate the opportunity to help you.  Since a good relationship depends upon mutual understanding and consideration, we wish to describe some of our policies.

About Appointments:

This office is usually open from 9:00 am to 5:00 pm Monday thru Friday.  The usual holidays are observed.

Evening appointments and Saturday appointments are generally discouraged, but are occasionally made under special circumstances.  If you need such appointments because of travel distance or other circumstances, please let the apointment secretary know and she will try to arrange a suitable time.  In the event of severe winter storms, please call ahead to be certain the office is open.

Treatment appointments are generally a "50 minute" hour.  Medication appointments are generally 15 to 20 minutes.  Evaluation appointments vary depending on the extent of the problems but generally are 1-2 hours.  Computer-assisted diagnostic appointments depend entirely on the specific diagnostic instruments and may vary from 15 minutes to 2 hours.  You will be told at the time of your appointment approximately how long you should plan to stay.

When your plans change for any reason and you cannot keep appointments, please call us and give us as much notice as possible.  You will be expected to pay for any session that is not cancelled at least 24 hours ahead of time.  For Monday appointments, please notify us by 9:00 a.m. that morning.  If necessary, please leave this message with our answering service.

Please inform us of any changes in address, phone numbers, place of employment, or insurance coverage.

Cost:

The cost of treatment will vary according to the utilized time period and the specific clinician providing the services.  Phone calls are subject to charge.  Please consult the clinician you are seeing for specific times and fees.

Prescription Refills:

Sound medical practice and federal regulations require careful control on all prescription drugs.  All of our written prescriptions are marked for a limited number of refills or for no refills.  Inquiries by a pharmacist about a refill should be made during our regular office hours so that we may have access to your record.  A check-up visit may be necessary before prescription medication refills are aurhorized.  Phone refills are subject to a charge, which is not reimbursable by insurance.

Confidentiality:

All communications between patient and clinician are confidential  No information can be released without your written consent or the consent of the custodial adult except in an extreme emergency.

Emergencies:

In the event of an emergency, please call the office.  The phone is answered 24 hours a day.  During regular office hours, the staff will direct you on how to proceed.  After office hours and on weekends, the phone will be answered by our answering service, who will relay your message to the clinician on call.
